It cannot be ruled out, as Russia undoubtedly has these "toys". If it starts losing decisively in the conventional war, it has gone so far that it will be difficult for it to withdraw from the war after suffering defeat. The use of tactical nuclear weapons is nothing unimaginable for Russian generals. They certainly learned it in school. Even the writer of these words, a former student of the Military Medical Academy, drew in the 1960s during tactics classes (up to the division level) with the use of such weapons. It was supposed to be provided by our allies at the time. Russia already knows that no one will love her. She means to be afraid of her! And such a tool is perfect for that.

Russia has a minister Lavrov for scaring, and I don't want to duplicate it. What should apply to the health service of a country of 37 million is minimal preparation in the event that something like this does happen. But I am not aware that there is at least one doctor dealing with this problem in Poland, and in any case I have not found any signs of such an action. I myself (although it seemed much more theoretical then than it is now) published an article in Military Physician No. 1 from 2020 entitled: "Medical aspects of the risk of radiation sickness in Poland" to which I refer. But I am only a military pensioner. Once, in the 1950s, in Poland, in the so-called patriotic circles: "one atomic bomb and we will return to Lviv again". It was then about an American Christmas ball. Today we do not want to take Lviv away from the Ukrainian brothers, but it is possible that the Russians are repeating this poem to themselves.
