Incorporating the Individualized Medical Care Plan (IPOM) into the IKP is another step in building a comprehensive tool for taking care of one's health. With this solution, patients covered by IPOM will find all the information about their treatment plan in one place. It's all about easy access to the schedule of tests, appointments and consultations.
The Individual Medical Care Plan is determined by the patient's primary care physician. After conducting an in-depth interview and analyzing the results of diagnostic tests, the primary care physician assesses the patient's current health status and determines a further treatment plan, which from now on is available on the Internet Patient Account. IPOM is an essential tool for coordinated care.
New quality of treatment
Coordinated care was introduced in Poland in October of the previous year. It deals with diagnosis and treatment, as well as prevention and patient education about selected diseases in the fields of cardiology, diabetology, endocrinology and pulmonology. Covers chronically ill patients over the age of 18. As part of coordinated care, the primary care physician collaborates with specialists, a primary care nurse and a dietician.
The patient, at every stage of treatment and appointment scheduling, is provided with a coordinator who schedules examinations, appointments and consultations ordered by the PCP, as well as educational and dietary advice. The coordinator does not have to be a doctor. It can be a nurse or a person from the registration desk. The coordinator is responsible for scheduling appointments and examinations, reminding the patient of appointments, updating the schedule, that is, he or she handles the organizational side of IPOM. The coordinator also searches for people who need individual care and invites them in for advice.
Internet Patient Account
The Internet Patient Account, on which IPOM is now available, is a free application from the Ministry of Health. It will quickly and securely check health information about yourself, your children or the person who authorized you to do so. More than 50% of adult Poles actively use their Internet Patient Account.
