There is a communication from the Ministry of Health on the fourth dose of vaccination against Covid-19

In a special announcement, the Minister of Health informs about the need to supplement the vaccination schedule in people who, following the administration of a booster dose, were included in the group of patients with immunodeficiency. Based on the recommendation of the Team for Protective Vaccinations of 18 May 2022, the Ministry of Health informs that in each such situation the next required dose of the vaccine should be administered.
People with impaired immunity who have received the primary vaccination schedule (no additional dose 3/3) and a booster dose should receive another dose of mRNA vaccine, i.e. Comirnaty Pfizer-BioNTech (0.3 ml) or Spikevax Moderna at the full dose (0 , 5 ml) at least 5 months after the last dose.
For vaccination, the same preparation of Comirnata (Pfizer-BioNTech) or Spikevax (Moderna), which received a booster dose, is recommended.
Additional information:
- it is essential to confirm the current vaccination status of the patient prior to administering the vaccine dose,
- the usefulness of serological tests for the assessment of the immune response to vaccination has not been established; serological testing to assess response to vaccination prior to administration of the vaccine is not recommended,
- when assessing the state of the patient's immune system, the severity of the disease, its duration, the patient's clinical condition, complications, comorbidities and any potentially immunosuppressive therapy should be taken into account,
- vaccine dose should be administered at least two weeks before starting or resuming immunosuppressive therapy, and the timing of vaccination against COVID-19 should take into account current or planned immunosuppressive therapy, as well as optimization of both the patient's clinical condition and response to the vaccine,
- immunosuppressed people, including those who have received a total of 4 doses of the COVID-19 mRNA vaccine, should be made aware of the risk of a weaker immune response to vaccination, and thus the need to continue to comply with the current COVID-19 protection rules. In order to provide additional protection to immunosuppressed patients, vaccination against COVID-19 should be recommended to people in their close environment (cocoon strategy),
- People who have received a booster dose instead of an additional primary dose, irrespective of the formulation used for the boost, should receive a dose of mRNA vaccine (Comirnaty or Spikevax full dose) at least 5 months after the third dose.
Principles of implementing revaccination schedules by the vaccination point
1. During vaccination, the doctor issues a referral for a booster dose. The doctor may issue a referral, assessing the patient's health condition and on the basis of information about the patient's indications for the next (additional) dose under the care of the patient, or on the basis of the patient's medical documentation. Before issuing a referral, the entries on the vaccination e-Card for previous doses should be verified.
