Patients with severe heart failure ineligible for heart transplantation with new therapeutic option

The introduction of a new guaranteed benefit in the Health Ministry's regulation on highly specialized services was based on the process of qualifying health care services as guaranteed benefits. The changes are aimed at providing access to left ventricular assist pump implantation surgery as a destination therapy, in patients with severe heart failure who are ineligible for heart transplantation as a destination therapy.
The expected effect of the "Mechanical heart support with a left ventricular assist pump in patients with severe heart failure who are ineligible for heart transplantation as destination therapy" benefit is to achieve sustained improvements in the management of severe heart failure in patients, who do not meet the eligibility criteria for heart transplantation, in particular, to improve hemodynamic parameters, such as an increase in ejection fraction, cardiac index, reduction in pulmonary artery wedge pressure and left ventricular end-diastolic dimension, and therefore clinically - to improve the functional class (NYHA), quality of life, increase life expectancy .
The introduction of the benefit is the implementation of the recommendation of the President of the Agency for Health Technology Assessment and Tarification No. 17/2022, dated June 2, 2022, which indicated the appropriateness of financing the assessed technology from public funds. Following the recommendation of the President of the Agency, on the basis of the order of the Minister of Health, work was carried out on determining the indications for the benefit in question and the conditions for its implementation.
