Implemented in 2024, the Comprehensive Care Program for Patients Over 65 with Diabetes is an innovative approach to improve the quality of life of seniors through multidisciplinary care and the implementation of modern technologies. By integrating various forms of support, the project focuses on minimizing the risk of diabetes-related health complications and promoting seniors' independence.
Key pillars of innovation include multidisciplinary care by specialists (a geriatrician, nutritionist, psychologist and physiotherapist) and the implementation of modern technology. An important element of the program is the introduction of continuous glycemic monitoring (CGM) systems, which allows patients to keep track of their blood sugar levels. This gives patients more control over their disease and allows them to adjust their health measures in real time. In addition, thanks to telemedicine solutions, doctors have the ability to remotely monitor patients' health. This solution is particularly beneficial for seniors, as it minimizes the need for frequent visits to the doctor, while providing constant medical monitoring.
Thanks to the program, patients can avoid many diabetes complications, such as heart disease, kidney failure, vision problems and cognitive impairment. This innovative program is a comprehensive solution that, by integrating modern technology and multifaceted care, helps the elderly live healthier and more independently. The introduction of continuous blood glucose monitoring and telemedicine allows not only better management of diabetes, but also an overall improvement in the quality of life of seniors.
