NHF: Coordinated care in primary care for patients under 18 years of age

According to the Team's recommendation, as of July 1, the implementation of health services under the entrusted budget of coordinated care for the population of patients under 18 years of age covered by a given provider, based on the declarations of choice submitted.
Although the scope of coordinated care examinations and consultations is dedicated to adults with chronic diseases, the Team felt that children should also benefit in medically justified cases.
Coordinated care in primary care
Coordinated care means many diagnostic and treatment options for the most common chronic diseases already with your family doctor. The primary care physician can order tests for patients (in medically justified situations) that were previously reserved for a specialist.
Treatment is based on an Individualized Plan of Medical Care (IPOM) and takes into account not only the performance of tests, but also consultations between the PCP and a specialist and additional consultations with, for example, a nutritionist.
