In this case, the Ministry of Health accepts only paper-based applications

There is a communication from the Ministry of Health on the submission of applications for reimbursement consent pursuant to Art. 39 of the Act of 12 May 2011 on the reimbursement of drugs, foodstuffs for particular nutritional uses and medical devices - regarding drugs imported from abroad as part of target import.

The Minister of Health informs that in connection with the entry into force of Art. 63 § 1 of the Act of June 14, 1960 Code of Administrative Procedure (Journal of Laws of 2021, item 735, as amended), as amended by the Act of November 18, 2020 on electronic deliveries (Journal U. of 2020, item 2320) applications for consent for reimbursement, pursuant to art. 39 of the Act of 12 May 2011 on the reimbursement of drugs, foodstuffs for particular nutritional uses and medical devices (Journal of Laws of 2021, item 523, as amended), hereinafter the "Reimbursement Act", brought on e-mail address will be left unrecognized.

Applications should be submitted on paper (to the address: ul. Miodowa 15, 00-952 Warsaw) or electronically (electronic inbox:/8tk37sxx6h/SkrytkaESP).

At the same time, the Minister of Health reminds that the applications for reimbursement consent pursuant to Art. 39 of the Act on the reimbursement should bear the signature:

  • handwritten - in the case of applications submitted on paper;
  • with a qualified electronic signature, a trusted signature or a personal signature - in the case of applications submitted in electronic form.
