The event is organized by the Association of Employers of Integrated Care Based on Values and the IFIC Polska Association in cooperation with the Polish Medicine Association.

During the conference, we will discuss the most important changes currently taking place in primary healthcare. The changes that we are all waiting for - representatives of healthcare entities, organizations associating specialists, representatives of patients and the entire group of specialists involved in the organization of the healthcare system. S we will buy one of the most important mechanisms that we have been implementing several years in primary care - coordinated care .
An integral part will be workshops for practitioners for practitioners :
- Workshop for managers: how to prepare a facility for coordination (first steps manual).
- Workshop for nurses: how to talk to a chronically ill patient.
- Workshop for coordinators.
During the two days of the conference, we will have the opportunity to discuss plans for organizing and coordinating activities aimed at patients in primary care.
The speakers will include people directly involved in the work of the POZ team, practitioners - people who implement the ideas of coordinated care in healthcare entities on a daily basis.
Together, we will talk about the role of a coordinator in POZ, about how to implement mechanisms of coordinated care in healthcare entities. We will discuss the role of nurses in the coordination of chronically ill patients, together we will discuss the importance of digital solutions.

The IFIC Polska association was established in April 2019 on the initiative of a group of stakeholders in the integrated care model, representing various healthcare-related professions, in order to jointly work to improve the quality and efficiency of the healthcare system in Poland.
The SPOIWO Employers' Association was established in June 2021. It brings together managers of medical entities from various industries, interested in the actual implementation of pro-quality and pro-efficiency solutions in health care.
