POZ: There is a draft ordinance on diagnostics within the entrusted budget

The changes are to ensure access, within the budget entrusted, to:
- diagnostic tests in the area of cardiology, diabetology, lung diseases, allergology and endocrinology - will allow for more effective detection of the most common chronic diseases,
- specialist consultations: cardiologist, diabetologist, lung diseases, allergist, endocrinologist - will facilitate comprehensive patient care without the need to refer them to a specialist doctor providing services as part of outpatient specialist care,
- comprehensive visits with an Individual Medical Care Plan - will improve care for chronically ill patients and contribute to the implementation of standards of care in chronic diseases covered by coordination,
- educational counseling conducted by nurses - will contribute to the improvement of the quality of care for chronically ill patients, as well as improve the complexity of care, involving primary care nurses (POZ) in the care,
- dietary advice - provided by dietitians - will be an important support for patients, among others with diabetes and atherosclerotic diseases.
The team for changes in primary health care recommended adding and financing within the budget entrusted the following diagnostic tests, specialist consultations and advice:
1. Biochemical and immunochemical tests:
1) anti-TPO (anti-thyroid peroxidase antibodies);
2) anti-TSHR (antibodies against TSH receptors);
3) antiTG (anti-thyroglobulin antibodies);
4) albuminuria (concentration of albumin in urine)
5) UACR (urine albumin/creatinine ratio);
6) BNP (NT-pro-BNP).
2. Ultrasound diagnosis:
1) Doppler ultrasound of the carotid arteries;
2) Doppler ultrasound of the vessels of the lower extremities;
3) ECHO of transstageal hearts.
3. Electrophysiological diagnostics:
1) exercise ECG (ECG stress test);
2) Holter ECG 24, 48 and 72 hrs. (24-48-72-hour ECG recording);
3) RR recorder (24-hour blood pressure recording)
4. Invasive diagnosis: fine needle aspiration biopsy of the thyroid gland (in adults).
5. Functional diagnosis:
1) Spirometry;
2) Spirometry with diastolic test.
6. Consultation and specialist advice:
1) consultations - a primary care physician - a specialist in a selected field (consultations using ICT systems or communication systems):
a) Consultation - a primary care physician - a specialist in the field of cardiology,
b) Consultation - GP doctor - specialist in the field of diabetes,
c) Consultation - a primary care physician - a specialist in the field of lung diseases,
d) Consultation - a primary care physician - a specialist in the field of allergology,
e) Consultation - a primary care physician - a specialist in the field of endocrinology;
2) Advice - specialist doctor - patient (outpatient visit):
a) Advice of a specialist in cardiology - patient,
b) Advice of a specialist diabetes specialist - patient,
c) Advice of a specialist lung disease specialist - patient,
d) Advice of an allergy specialist - patient,
e) Advice of a specialist in endocrinology - patient.
7. Tips
1) Comprehensive advice - Individual Care Plan;
2) Diet advice;
3) Educational advice.
