The Ministry of Health appoints the Team for the implementation of coordinated care in POZ

The Team consists of:
1) Chairman - dr hab. n. med. Agnieszka Mastalerz-Migas - attorney, national consultant in the field of family medicine;
2) Vice-chairmen:
a) Maciej Karaszewski - Deputy Director of the Health Care Department at the National Health Fund,
b) Paweł Żuk – President of the Management Board of Centrum Medyczno-Diagnostyczne Sp. z o.o.;
3) members:
a) Michał Dzięgielewski – director of the Department of Medicine in the Ministry of Health,
b) Grzegorz Krupa – representative of the Healthcare Department at the Ministry of Health,
c) Piotr Węcławik – director of the Department of Innovation in the Ministry of Health,
d) Andrzej Zakrzewski - director of the Warmian-Masurian Regional Branch of the National Health Fund,
e) Andrzej Sarnowski - Director of the Division (DIII), Deputy Director for SIM development and implementation at the e-Health Centre,
f) Justyna Pancerow - Director of the Department of Implementation of the e-Health System at the e-Health Centre,
g) Tomasz Zieliński – vice-president of the Federation of Health Care Employers' Associations, the Zielonogórskie Agreement,
h) Beata Ostrzycka – national consultant in the field of family nursing,
i) Mariola Łodzińska – vice-president of the Supreme Council of Nurses and Midwives,
j) Aleksander Biesiada - plenipotentiary of the Main Board of the Polish Society of Family Medicine for Innovation and Development,
k) Agnieszka Brześcińska – vice-president of the National Association of Family Midwives;
4) secretary - representative of the Healthcare Department in the Ministry of Health.
The tasks of the Team include:
1) monitoring the process of implementing coordinated care in primary health care;
2) developing evaluation indicators for the implementation of coordinated care in primary health care;
3) development of a set of indicators for assessing the quality and effectiveness of care in primary health care;
4) developing recommendations for the assumptions of legislative solutions regarding coordinated care in primary health care, taking into account the comments made by service providers;
5) developing a strategy for the development of coordinated care in primary health care, taking into account the pediatric population;
6) designing digital solutions to improve the comprehensiveness of care in primary health care;
7) developing proposals for the rules of cooperation between primary health care units and outpatient specialist care units;
8) performing other tasks assigned by the minister responsible for health related to the subject of the Team's activity.
